Create data collection templates for monitoring student progress
Data collection templates provide a format for record keeping that both teachers and schools need to monitor student progress. Whereas a school may only be monitoring selected data based on their school improvement plan, teachers need to monitor the objectives they are responsible for teaching. It is worth noting that, before the Voluntary State Curriculum (VSC), Maryland assessed at the indicator level. Now that the content standard format has been revised, Maryland assesses at the objective level. Though a school district may add additional objectives to be taught, the state will only assess what students are expected to know and do as written in the Maryland Content Standards.

The following data collection templates have been created to support schools in monitoring student progress toward proficiency on the Maryland Content Standards.

1. Unit Monitoring Report

The Unit Monitoring Report can be used by teachers as a grade sheet to align what they are teaching to the Maryland content standard objectives. A teacher or team of teachers would look at their district curriculum unit of study and, using the templates below, identify which Maryland content standard objectives they were teaching as part of that unit. By placing a check beside the objective you will be monitoring during a unit of study or grading period, you can print out a grade book customized for your use. You can also download the Excel spreadsheets onto your desktop to edit and use as you wish.

2. Individual Student Monitoring Report

The Individual Student Monitoring Report provides space for assessment/observation and diagnostic information for all of the assessed objectives in reading and mathematics at a specific grade level. The report might be used by a teacher to monitor progress of a whole class or of a smaller group of underperforming students. It might also be used by students to track their own progress on the objectives they are expected to learn and will be assessed on that year in mathematics and reading. It might be part of a student's portfolio or reading journal that also included the student work aligned to the objectives being monitored.
